New Patient Information

To make your first visit as smooth as possible, please bring a photo ID, your insurance card, a list of current medications (including dosages), and any relevant medical or mental health records. Please arrive 15 minutes early to complete intake paperwork. During your first visit, your provider will review your history, discuss your concerns and goals, and answer any questions you may have.

Join your appointment a few minutes early using the link provided. Please have your ID and medication list handy, just as you would for an in-person visit.
